1. I acknowledge that the events are run by the coordinator and volunteers of the SBCC, who give freely of their time. Therefore, I acknowledge that respectful communication will be maintained at all times. In the event of my/my child's late attendance or need for early pickup, I will notify the event coordinator.
2. I acknowledge that there will be a 30-minute grace period if the participant is not picked up by the stated completion time of the event. If contact cannot be made with parents/guardians, the authorities will be contacted.
3. I acknowledge that SBCC has a zero-tolerance policy for harassment and bullying. If I/my child is identified in such behavior, after discussion with staff, I/my child may be asked to leave the event early. Repeated offenses may result in suspension or expulsion from the group.
4. I acknowledge that I/my child will not engage in any inappropriate or illegal activity and will stay with the group at all times. We will be socially conscious during external activities and show self-respect with appropriate language and manners, extending this respect to the coordinator, volunteers, and the general public.
5. I (parent/guardian) acknowledge that I may need to sign additional waivers for particular activities held on or off base.
6. I (parent/guardian) acknowledge that I must sign my child in and out of every event.
7. At least one parent/guardian registered in this form has legal custody over the child.
8. For medical emergencies, I allow the medical team of this organization to take care of my child and transport them to the nearest hospital by ambulance if necessary.
9. I release this organization and its volunteers from any and all liability for accidents or injuries to the child during organization-related events.
10. Costs for Youth Group activities are subsidized under the SBCC Membership. I understand that I will pay full associated costs for individual activities if I do not hold a current SBCC Membership.
